New Patient Packet
Before your initial visit, we will need you to complete a new patient packet provided by our office. Filling out the enclosed packet prior to your scheduled appointment will reduce processing time for both you and the practice. Your patient packet will also provide us the information needed to best serve your medical needs. Please fill out the packet completely and do not hesitate to call us at (270) 538-5700 with any questions you may have.

Once you have downloaded and printed the new patient packet, please fill out all forms completely and fax to our office at 270-538-5701. After receiving your packet, we will register your information in our system in preparation for your first visit.
